ALL Coaches as well as all team moms, umpires, and volunteers, must complete a application and Background Check.

2010 Coach's App & Background Check

 

This form must be filled out and signed by parent or legal guardian, and turned in with player registration form, copy of birth certificate, and registration fee by the Coach Protecting the player.
